The board and its employees and agents who authorize the self-administration of medication and treatment for diabetes in compliance with the provision of this policy, shall not be liable in any action for any injury resulting from the self-administration of medication by a student, when authorized to do so by a licensed health professional and approved by the parent/guardian.

As the parent/guardian of,                                                                     I hereby acknowledge that the district incurs no liability for any injury resulting from the self- administration of medication, the incorrect self-administration of medication, or the failure to self-administer medication, for the treatment for diabetes, and agree to indemnify and hold the board and its employees and agents harmless against any claims relating to the self-administration of medication and treatment for diabetes pursuant to this policy.
